Diabetes Test
1. The test is for yourself, or you are the parent or guardian of the person taking the test (defined as “you” below).
2. You are over the age of eighteen years.
3. You are not pregnant or breastfeeding.
4. You do not have diagnosed diabetes.
5. You must not be known to be suffering from anaemia or have had your spleen removed.
6. You understand that if less than 250μL of blood is provided you may not receive a result.
7. You understand that if you don’t turn the capped blood tube upside down 10 times after collecting the sample you may not receive a result.
8. You understand that the sample should be taken on a Monday, Tuesday or Wednesday and the sample posted the same day.
9. You understand that personal and special category data is stored in accordance with our privacy policy and UK Data Protection laws
10. You understand that if you do not complete and attach the small label to the blood sample tube then you will not receive any results.
